Discussing Placement and Level of Care.
We welcome enquiries at any stage, whether a decision is weeks away or a discharge is imminent. We can advise on current availability, level of care, daily routine and the placement process, and will state directly where an individual requires a higher level of care than a licensed adult family home may provide.
Information Required at Initial Enquiry
No preparation is necessary. However, having the following information available enables a substantive assessment during the initial call.
Information We Will Request
- The individual requiring care, and their age
- Level of assistance required with bathing, dressing, oral intake, toileting and mobility
- Cognitive status, elopement risk and any behaviours requiring a planned approach
- Current medication regimen, diagnoses, and dietary or swallowing requirements
- Current setting home, hospital, rehabilitation or another facility
- Anticipated timeline for admission
Questions Worth Putting to Any Provider
- Is there a current opening, and in which home?
- Who provides the daily care, and how consistent is the team?
- What are the overnight staffing arrangements?
- How is an increase in care needs assessed and accommodated?
- What is the frequency and method of family communication?
- What is the fee, and what does it include?
Frequently Asked Questions
May we tour the home before deciding?
Yes, and we recommend it. Prospective residents and families are invited to tour the home, meet the caregiving team and view the accommodation. Please phone to arrange a time so that the visit does not coincide with scheduled personal care.
Are visiting hours restricted after admission?
No. These are residents' homes rather than institutional settings. Families are welcome to visit, share a meal and take a resident out. Advance notice is appreciated so that visits do not coincide with personal care or rest periods.
What happens if care needs increase after admission?
Increasing need is anticipated. We reassess and revise the individual service plan accordingly, and can accommodate substantially greater assistance, including full support with every activity of daily living. Where a resident comes to require skilled nursing beyond the scope of a licensed adult family home, we advise the family early and assist in planning the transition rather than deferring to a crisis.
May residents bring their own furniture and decorate?
Yes. Rooms are furnished, and residents may bring their own furniture and belongings, display pictures and paint the room. Familiar personal objects are of particular benefit to residents with cognitive impairment.
Do residents retain their own physicians?
Yes. We coordinate with each resident's existing physicians, specialists, therapists and home-health providers rather than replacing them, and arrange transportation to medical appointments.
What is the cost of placement?
Cost is determined by the level of care required and the home in which a placement is available, and is therefore quoted individually rather than published. We will set out the fee and what it includes at the point of enquiry.
How quickly can an admission proceed?
Where an opening exists and documentation is complete, admission can proceed quickly; we work routinely to hospital discharge timelines. Availability, rather than process, is the limiting factor.
